Consent and Permission Form Language
NEW CONSENT FORM LANGUAGE:
New sample language focused on safety and hygiene will appear on the IRB informed consent, parent permission, and assent forms located in Mentor IRB/Documentation or on TC IRB's website.
OFF-CAMPUS RESEARCH AND NEW SITE PERMISSION FORM:
- Off-campus research is defined as research conducted at an external location from the Teachers College campus. These research projects may include but are not limited to: field research; laboratory research at universities, colleges, and other agencies; inter-institutional collaborative projects; or analysis of existing data.
- A new off-campus, in-person research site permission form is available in Mentor IRB/Documentation or by downloading it here. This new form includes language related to COVID-19, with safety and hygiene expectations when engaging off-campus.
- For off-campus studies, researchers should get an external Site Permission Form signed by the site coordinator (download it here).
- If a researcher has a question about conducting off-site, in-person data collection should email IRB@tc.edu or visit TC IRB’s Updates page.
Researchers engaging in off-campus, in-person data collection may also want to consult the "Researcher Safety Assessment Guide." This document provides insights into how to assess the risk of a situation and ways to mitigate the risk.
